Officer Nomination Form 2021

The 2021 National Conference will be an extra special one — we are electing a new President and Vice President! Use this form to declare your candidacy to the Nominating Committee.

Important things to note:

1. Forms must be received at least 90 days before the conference, i.e. by April 24, 2021.

2. Nominations SHALL NOT be taken from the floor.

3. The President and Vice President run as a ticket. Each form must include names and contact information for both candidates.

4. Candidates will be reviewed for eligibility by the Nominating Committee.
